Club affiliation form - all affiliated clubs will receive this from their District.  Affiliation is through the District.  New club ?  Contact info@ontariobadminton.on.ca.

Membership form - to join the OBA a player must belong to an affiliated club.  Membership registration is available on line.

Coach registration - no fee (except for insurance, payable only once as a member, coach or official) - see attachment

Official registration - no fee (except for insurance, payable only once as a member, coach or official) - see attachment
